29 CFR 1926.20(b)(2): A program was not provided that provided for the frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons: a) Job site, at/near 537 Ohio River Boulevard, Pittsburgh, PA: On or about July 9, 2025 during roofing activities, the employer did not ensure a system was implemented to identify and correct safety hazards. Deficiencies noted during the inspection included, but were not limited to, not having the electrical service drop blanketed, boards being left on the ground with nails sticking up, using a ladder with bent rungs, not using fall protection, using a ladder that was set up to steep, ladder labels were illegible and not using eye/face protection while operating nail guns.
