29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:   a) Job site, at/near 537 Ohio River Boulevard, Pittsburgh, PA: On or about July 9, 2025 during roofing activities, the employer did not ensure an employee used appropriate eye/face protection while operating a nail gun.

29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provided for an alternative fall protection measure:  a) Job site, at/near 537 Ohio River Boulevard, Pittsburgh, PA: On or about July 9, 2025 during roofing activities, the employer did not ensure employees used appropriate fall protection systems/devices where exposed to fall hazards of up to at least approximately 10 feet.

29 CFR  1926.20(b)(2): A program was not provided that provided for the frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons:    a) Job site, at/near 537 Ohio River Boulevard, Pittsburgh, PA: On or about July 9, 2025 during roofing activities, the employer did not ensure a system was implemented to identify and correct safety hazards. Deficiencies noted during the inspection included, but were not limited to, not having the electrical service drop blanketed, boards being left on the ground with nails sticking up, using a ladder with bent rungs, not using fall protection, using a ladder that was set up to steep, ladder labels were illegible and not using eye/face protection while operating nail guns.
