29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye: (a) 200 West Orange Street, Lititz, Pennsylvania - An employee suffered work-related injuries to both arms on June 5, 2025 after an approximately 13-foot fall from a ladder. The injury required in-patient hospitalization, and the employer did not report this in-patient hospitalization to OSHA within 24 hours. No abatement certification or documentation required
