29 CFR 1910.178(k)(1):The brakes of highway truck(s) were not set and wheel chocks were not placed under the rear wheels to prevent truck(s) from rolling when boarded by powered industrial vehicles: a) On July 7, 2025, and times prior, at Old Dominion Freight Line, Inc, 4530 15th Ave N, Fargo, North Dakota, in the loading dock area, employees operated forklifts while loading, unloading and moving material. The employees boarded unattached trailers at the loading docks with forklifts and did not secure the trailers with wheel chocks to guard against rolling forward, exposing them to crushing by hazards.

29 CFR 1910.178(a)(4):Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: a) On July 7, 2025, and times prior, at Old Dominion Freight Line, Inc, 4530 15th Ave N, Fargo, North Dakota, in the loading dock area, employees loading, unloading and moving material to trailer bays operated forklifts with 6-foot fork extensions without the manufacturers written approval. This could result in tip overs, exposing employees to crushed by or struck by hazards. b) On July 7, 2025, and times prior, at Old Dominion Freight Line, Inc, 4530 15th Ave N, Fargo, North Dakota, in the loading dock area, employees loading, unloading and moving material to trailer bays operated a Toyota forklift with MSI 8-foot forks without the manufacturers written approval. This could result in tip overs, exposing employees to crushed by or struck by hazards. c) On July 7, 2025, and times prior, at Old Dominion Freight Line, Inc, 4530 15th Ave N, Fargo, North Dakota, in the loading dock area, employees loading, unloading and moving material to trailer bays operated Toyota forklifts with a Rice Lake weighing systems forklift scale without the manufacturers written approval. This could result in tip overs, exposing employees to crushed by or struck by hazards.
