29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Auto Bagger location - On or about June 30, 2025, the employer did not provide effective guarding to protect employee(s) working on or near the Auto Bagger (Pacmac Model 2015V2 Vertical Form, Fill & Seal machine /SN# 2015B-0381). An employee left hand came in contact with the unguarded portion of the machine, which subsequently caused an amputation to the left hand middle and ring fingers.
