The employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative. a) Prodex, Inc. 436 Adams Street, Red Hill, PA 18076: On or about July 10, 2025, the employer failed to establish a program of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials, a minimum distance of 20 feet or by a noncombustible barrier at least 5 feet high having a fire-resistance rating of at least one-half hour. a) Prodex, Inc. 436 Adams Street, Red Hill, PA 18076: On or about July 10, 2025, employees were exposed to fire hazards in that oxygen and propylene tanks were stored on a cart without a noncombustible barrier at least 5 feet high having a fire-resistance rating of at least one-half hour.
