A) Jobsite: 2000 Brown St. Philadelphia, PA 19130 - On or about July 11, 2025, the employer did not ensure employees performing roofing activities were protected from falls by guardrail systems, safety net system, or personal fall arrest system. The employees were performing roofing activities from the flat roof of a one story residential building without the use of fall protection, thereby exposing employees to fall hazards of approximately 10 feet.

Philadelphia, PA 19130 - On or about July 11, 2023, the employer did not ensure that an employee performing roofing activities from the first floor roof of a residential building were trained in recognizing fall hazards, thereby exposing the employee to fall hazards of approximately 10 feet above ground level.
