29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: a) New construction located at 7293 Maple Brook Lane, Flowery Branch, GA 30542, Roof: On or about 07/11/2025, the employer exposed employees to fall hazards of 19-feet to the lower ground level, in that a personal fall protection systems were not used by employees engaged in roofing activities which included loading the roof with shingles on a roof pitch of 7/12.

29 CFR 1926.552(a)(4): Hoisting ropes were not installed in accordance with the wire rope manufacturers' recommendations. a) At the new construction site located at 7293 Maple Brook Lane, Flowery Branch, GA 30542, Roof: On or about July 11, 2025, the employer exposed employees to potential struck-by hazards while operating a TranzSporter TS250 material hoist, in that a nylon rope was used in place of the manufacturer-required wire rope, which was not in accordance with the hoist's design specifications and manufacturer instructions.
