29 CFR 1910.28(b)(11)(ii): 29 CFR 1910.28(b)(11)(ii): The employer did not ensure that each flight of stairs having at least 3 treads and at least 4 risers is equipped with stair rail systems and handrails as described in 29 CFR 1910.28 Table D-2-Stairway Handrail Requirements. a. Customer Cap building: On or about July 10, 2025, and at times prior, employees were exposed to fall hazards while accessing and egressing the flight of stairs with five treads and six risers. The stair rail system was missing a midrail on both sides.

29 CFR 1910.303(g)(1):Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment: a. Tire storage building: On or about July 10, 2025, and at times prior, employees were exposed to electrical shock hazards from being unable to deenergize equipment in the event of emergency due to a boxes blocking the electrical panel.
