(a) Worksite - 2nd floor: On or about 7/14/2025, employees were exposed to fall hazards of approximately 13 feet to the ground level, while working near the edge of the building receiving a media blasting hose without fall protection. (b) Worksite - 3nd floor: On or about 7/14/2025, employees were exposed to fall hazards of approximately 22 feet to the ground level, while working near the edge of the building lowering a media blasting hose, without fall protection.
