29 CFR  1910.140(c)(18): The employer did not ensure that personal fall protection systems are inspected before initial use during each workshift for mildew, wear, damage, and other deterioration, and defective components are removed from service.  a) Worksite, 123 Beach Ave., Wading River, NY- An employee was cutting tree branches at a height of approximately 30' using a harness and lifelines that showed visible signs of wear, damage and deterioration. The fall protection equipment was not inspected prior to use; on or about 7/14/25.  Note:  In addition to abatement certification the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.
