29 CFR 1904.39(a)(1): The employer did not report within 8-hours the death of an employee resulting from a work-related incident: a) On or about 07/11/2025; where an employee died of an unwitnessed electrocution at a worksite located at , Buffalo, New York. The employer did not notify OSHA of the death. OSHA became aware of this fatality through a phone call from the Medical Examiner and later the employer on 07/14/2025. NO ABATEMENT CERTIFICATION REQUIRED
