29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. a) On or about July 2, 2025, at 1700d, Greenbrier Dear Rd, Anniston, AL 36207, an employee suffered a work-related injury and was hospitalized. The employer was aware of the in-patient hospitalization on July 3, 2025. The employer notified OSHA on July 8, 2025.
