29 CFR  1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  (a) David Ramirez DBA David Ramirez failed to document abatement within 10 calendar days after the abatement date that each of the following cited violations had been corrected.  Citation 1, Item 1 a serious violation of 29 CFR 1926.20(b)(2)with an abatement date of August 29, 2025.  Citation 1, Item 2 a serious violation of 29 CFR 1926.102(a)(1) with an abatement date of August 29, 2025.  Citation 1, Item 3 a serious violation of 29 CFR 1926.501(b)(13) with an abatement date of August 29, 2025.  Citation 1, Item 4 an other-than- serious violation of 29 CFR 1926.503(b)(1) with an abatement date of August 29, 2025. 29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles.  (a) On or about July 14, 2025, at N17 W29650 Caldicot Circle, Delafield, WI 53018, the employer did not ensure employees conducting roofing operations, using hand tools and nail guns were wearing eye protection. This exposed the employees to struck by hazards and eye injuries.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  (a) On or about July 14, 2025, at N17 W29650 Caldicot Circle, Delafield, WI 53018, employees installing a roof on a new residential home with a 6:12 pitch were not using fall protection. This exposed the employees to a fall hazard of approximately 15 feet 4 inches from the roof eve to the ground below on the back side of the home.
