29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. a.) Parking Lot, Heller's Gas, 16590 State Route 3, Watertown, NY 13601, on or about June 24, 2025; At approximately 2pm an employee was operating a crane from the parking lot and the load swung resulting in a 500-gallon steel tank hitting the employee's right knee cap. The employee was hospitalized for treatment. OSHA was not notified of the hospitalization until July 15, 2025.
