29 CFR  1926.416(a)(1): The employer permitted employees to work in close proximity to energized, unguarded, and ungrounded electric power circuits that the employee could contact in the course of their work: (a)(LOCATION)(IDENTIFY SPECIFIC EQUIPMENT AND/OR CONDITIONS)(DESCRIBE HAZARDS WHERE NECESSARY)  a) On or about July 14, 2025, at a worksite located at 2619 Wisconsin Ave NW, Washington, DC; parking deck,  The employer did not properly de-energize the switchgear while employees were running cable, which led to an electrical arc. As a result, one employee sustained electrical burns and also experienced smoke inhalation.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.  This documentation may include but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR  1926.416(a)(3):The employer began work without determining if employees could contact any part of an energized circuit and did not post warning signs and advise employees of the location of the circuit, hazards, and protective measures they need to take.  (a)(LOCATION)(IDENTIFY SPECIFIC EQUIPMENT AND/OR CONDITIONS)(DESCRIBE HAZARDS WHERE NECESSARY)  a) On or about July 14, 2025, at a worksite located at 1776 K Street NW, Washington, DC, the employer failed to develop comprehensive safety procedures and did not conduct thorough training for employees on the hazards associated with working withing an energized switchgear high-voltage equipment (480V).  They  also failed to provide proper personal protective equipment (PPE), including insulated gloves, voltage-rated tools, and arc-rated clothing. As a result of these oversights, a serious incident occurred in which an employee suffered electrical burns from an electrical flash.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.  This documentation may include but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR  1926.950(b)(1)(i):Each employee shall be trained in, and familiar with, the safety-related work practices, safety procedures, and other safety requirements in this subpart that pertain to his or her job assignments.  a) On or about July 14, 2025, at a worksite located at 1776 K Street NW Washington DC, P1 parking deck, the employer allowed employees to work on energized electrical equipment (480V) without implementing essential electrical safety procedures. There was an absence of an job safety analysis, which should have identified the potential hazards associated with working on high-voltage electrical systems. Furthermore, employees did not receive adequate training regarding the dangers associated with electrical equipment, including the risks of electrical shocks and arc flashes.    The lack of an risk assessment, proper development and training procedures resulted in a serious incident where one employee sustained electrical burns due to an arc flash while attempting to run cables  into the switchgear. This incident resulted in the employee suffering second-degree burns.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification. This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

The job briefing required by § 1926.952 shall cover information on special system conditions affecting employee safety, including the location of energized equipment in or adjacent to the work area and the limits of any deenergized work area.  a) a) On or about July 14, 2025, at a worksite located at 1776 K Street NW Washington DC, P1 parking deck, The employer failed to  protect employees of a hazardous electrical environment while working around energized electrical equipment, resulting in a arc flash event. This incident occurred because the job briefing, which was supposed to include critical information about the location of energized equipment, was not conducted before the work commenced. As a result, the incident resulted in the employee suffering electrical burns.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.
