29 CFR 1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye: On or about July 9, 2025, at 4500 Parkway Drive, Brookfield, OH 44403, the employer failed to report hospitalization of an employee who was injured on June 27, 2025 when a metal coil on the re-coiler struck them while banding within 24 hours.
