29 CFR 1926.501(b)(10):Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall protection plan which meets the requirements of paragraph (k) of section 1926.502: (a) On or about July 15, 2025, employees doing roof work a new commercial building were exposed to fall hazards to the lower level.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder was used to gain access: (a) On or about July 15, 2025, employees doing roof work a new commercial building were exposed to fall hazards to the lower level. Front East : Employees were exposed to approximately 15-feet fall hazard.
