29 CFR  1926.501(a)(2):  The employer did not determine if the walking/working surfaces on which its employees were working had the strength and structural integrity to support employees safely. Employees were allowed to work on those surfaces that did not have the requisite strength and structural integrity:  On or about July 15, 2025,  on the section of building which was 60 feet by 90 feet (office), employees setting roof trusses were exposed to fall hazards where prefabricated wood roof trusses and exterior building walls were not adequately braced during installation. The roof trusses (approximately 12 trusses) and exterior building walls which were set on July 15, 2025, were not adequately braced such that there was inadequate temporary bracing for the structure.  Employees working on these walking / working surfaces were exposed to falls such that they did not have the requisite strength and structural integrity.  On or about July 15, 2025, on the section of building which was 60 feet by 100 feet (shop), employees setting roof trusses were exposed to fall, struck by, and caught between hazards where prefabricated wood roof trusses were not adequately braced during installation.  The roof trusses (approximately 4 trusses) and exterior building walls which were set on July 15, 2025, were not adequately braced such that there was inadequate temporary bracing for the structure.  These conditions contributed to the collapse of the 4 installed trusses where two employees were injured and transported to the hospital.  Employees working on these walking / working surfaces were exposed to falls such that they did not have the requisite strength and structural integrity.  On or about July 22, 2025,  on the section of building which was 60 feet by 90 feet (office), employees setting roof trusses were exposed to fall, struck by, and caught between hazards where prefabricated wood roof trusses were not adequately braced during installation. The roof truss which were set on July 15, 2025 (approximately 46 trusses) and exterior building walls and roof trusses which were set on July 22, 2025 (approximately 34 trusses) , were not adequately braced such that there was inadequate temporary bracing for the structure.  Employees working on these walking / working surfaces were exposed to falls such that they did not have the requisite strength and structural integrity.  On or about July 23, 2025, on the section of building which was 60 feet by 100 feet (shop), employees setting roof trusses were exposed to fall, struck by, and caught between hazards where prefabricated wood roof trusses were not adequately braced during installation.  The roof trusses (approximately 51 trusses) and exterior building walls which were set on July 23, 2025, were not adequately braced such that there was inadequate temporary bracing for the structure.  These conditions contributed to the collapse of all the 50 installed trusses where two employees were injured and transported to the hospital.  Employees working on these walking / working surfaces were exposed to falls such that they did not have the requisite strength and structural integrity.
