29 CFR  1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about July 15, 2025, at jobsite located at 202 N. Maples St., Elmhurst IL 60126, employees were exposed to eye injuries while engaged in roofing activities operating a pneumatic nail gun.  Eye protection was not being utilized while employees were engaged in this activity.    Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d)

29 CFR  1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system:    On or about July 15, 2025, at jobsite located in 202 N. Maple Ave., Elmhurst, IL 60126, employees were exposed to fall hazards in excess of six feet while engaged in residential roofing activities. Fall protection was not being utilized while performing roofing activities.

29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about July 15, 2023, at jobsite located in 202 N. Maple Ave., Elmhurst, IL 60126 , employees performing roofing activities were exposed to fall hazards in excess of six feet with inadequate protection when the employer failed to ensure they were adequately trained to recognize and minimize the hazards associated with falls.  Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
