29 CFR  1910.36(d)(1): Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge:   On or about July 16, 2025, Happy Tails Thrift Shop, did not ensure employees are able to open the exit route door from the inside at all times without keys, tools, or special knowledge.  Employees working in the facility were exposed to a potential fire hazard from the emergency exit door being locked from the inside with a deadbolt lock and a bungie cord used to keep the door closed requiring  special knowledge to open in the event of an emergency.

29 CFR  1910.304(b)(3)(ii)(C)(3): Each cord set, attachment cap, plug, and receptacle of cord sets, and any equipment connected by cord and plug were not visually inspected before use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage, and equipment found damaged or defective was not removed from use until repaired:  a) On or about July 16, 2025, the employer Happy Tails Thrift Shop had a outdoor electrical receptacle installed on the side of the building that was used to plug in an 120-volt extension cord to plug into Connex Containers CBHU 6214737 and MSCU 4808863, had a 120-volt extension cord had an extension cord with a three-prong grounded plug had the exterior wire cover pulled away exposing the 3 interior wires.    b) On or about July 16, 2025, the employer Happy Tails Thrift Shop, had an outdoor electrical receptacle installed on a side of a pole behind the donation tent  had a 120-volt extension cord that had a plug that was damaged with a visible crack.  This cord was being used to power Connex Containers MSCU 480886 and OOCL 00780446.  Any reference to MSCU connex 4808863 or MSCU connex 480886 is hereby stricken from Citation 2, Item 2 (a); Citation 2, Item 3 (a); and Citation 2, Item 3(b).

29 CFR  1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):   a) On or about July 16, 2025, the employer Happy Tails Thrift Shop, exposed employees to an electrical shock hazard of 120-volts from portable extension cords outdoors; the employer did not ensure to visually inspect the portable cord and plug connection for external defects and for evidence of possible internal damage that was plugged into a outlet on the outside of the building.    b) On or about July 16, 2025, the employer Happy Tails Thrift Shop,  exposed employees to an electrical shock hazard of 120-volts from portable extension cords outdoors; the employer did not ensure to visually inspect the portable cord and plug connection for external defects and for evidence of possible internal damage that was plugged into a outlet on the outside of the building.

29 CFR  1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:   a) On or about July 16, 2025, the employer Happy Tails Thrift Shop did not hard wire the electrical requirements for the Connex container CBHU 6214737 thus exposing employees working in the container to fire hazards. The employees were using extension cords that were daisy chained to provide electricity needs to the container.  b) On or about July 16, 2025, the employer Happy Tails Thrift Shop did not hard wire the electrical requirements for the Connex container MSCU 4808863 thus exposing employees working in the container to fire hazards. The employees were using extension cords that were daisy chained to provide electricity needs to the container.  c) On or about July 16, 2025, the employer Happy Tails Thrift Shop did not hard wire the electrical requirements for the Connex container MSCU 480886 thus exposing employees working in the container to fire hazards. The employees were using extension cords that were daisy chained to provide electricity needs to the container.  d) On or about July 16, 2025, the employer Happy Tails Thrift Shop did not hard wire the electrical requirements for the Connex container OOCL 00780446 thus exposing employees working in the container to fire hazards. A Flexible (120 Volt) was attached to an extension cord and then run into a Connex Shipping Container OOCL 00780446, under the trailer door used for lighting the inside of the container.  b) On or about July 16, 2025, the employer Happy Tails Thrift Shop had flexible cords running through a Connex Shipping Container.  A Flexible (120 Volt) was attached to an extension cord and then run into a Connex Shipping Container MSCU 480886, under the trailer door used for lighting the inside of the container.  Any reference to MSCU connex 4808863 or MSCU connex 480886 is hereby stricken from Citation 2, Item 2 (a); Citation 2, Item 3 (a); and Citation 2, Item 3(b).
