29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  At the temporary jobsite located at 1137 W. Trek Cir., Goddard, KS 67052, employees engaged in residential framing activities were exposed to fall hazards of approximately 12 feet and the employer did not ensure the aluminum ladder was extended at least 3 feet above the eave.

29 CFR  1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical agents:  At the jobsite located at 1137 W. Trek Cir., Goddard, KS 67052, employees engaged in residential roofing activities with the use of pneumatic nail guns were exposed to struck-by hazards, in that eye protection was not utilized.  Juan Olvera was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1), which was contained in OSHA inspection number 1783320, citation number 1, item number 1, and was affirmed as a final order on 03-04-2025, with respect to a workplace located at 5017 Emerald Ct, Maize KS.  Juan Olvera was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1), which was contained in OSHA inspection number 1709606, citation number 1, item number 1, and was affirmed as a final order on  01-29- 2024, with respect to a workplace located at 5005 N Remington St, Bel Aire, KS.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501  At the temporary jobsite located at 1137 W. Trek Cir., Goddard, KS 67052 employees engaged in residential roofing  activities were exposed to fall hazards in the following instances:  1.  At the southeast side of the residential structure a roofer was working at heights of approximately 15 feet without fall protection.and the employer did not ensure the employees were protected by any form of fall protection. 2. At the northwest side of the residential structure a roofer was working at heights of approximately 12 feet without fall protection.and the employer did not ensure the employees were protected by any form of fall protection.  Juan Olvera was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1783320 citation number 1, item number 2, and was affirmed as a final order on 03-04-2024, with respect to a workplace located at 5017 Emerald Ct, Maize KS.
