Site: 493 Hinsdale Street, Brooklyn NY 11207 On or about 7/16/25 a) Employees worked from a 3-tier tubular frame welded scaffold they were exposed to to a fall hazard of approximately 20 feet. The scaffold was not fully planked or decked.

29 CFR 1926.451(e)(1):When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access. Site: 493 Hinsdale Street, Brooklyn NY 11207 On or about 07/16/2025 a) Employees working from the 2-tier of a 3-tier tubular frame welded scaffold were not provided with safe means of egress. The employees used the scaffold frames to access their work area.
