29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  (a)  On or about July 17, 2025, employees conducted roofing work on a new industrial building at 391 North Port Washington Road in Grafton, Wisconsin, without the use of fall protection. This exposed employees to fall hazards of approximately 22.5 feet at the peak and approximately 13.75 feet at the eve of the roof.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a)  On or about July 17, 2025, employees conducted roofing work on a new industrial building at 391 North Port Washington Road in Grafton, Wisconsin, were not trained in the recognition of fall hazards and the methods to implement fall protection systems. This exposed employees at the job site to fall hazards of approximately 22.5 feet at the peak and approximately 13.75 feet at the eve of the roof.

29 CFR  1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  (a)   On or about July 17, 2025, employees conducted roofing work on a new industrial building at 391 North Port Washington Road in Grafton, Wisconsin, utilized a ladder for access/egress to a roof area that did not extend at least 3 feet above the landing, exposing an employee to a fall hazard of approximately 13.75 feet.

Employees were exposed to fall hazards while installing a new roof on the building.
