29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.  Instance A:  On or about July 17, 2025, employees in the fabrication shop were utilizing a drill press that was not guarded in a way to protect from point of operation and rotating part hazards.  Instance B:  On or about July 17, 2025, employees in the fabrication shop were utilizing an electric Pipe Nibbler Press that was not guarded in a way to protect from point of operation and ingoing nip point hazards.  Instance C:  On or about July 17, 2025, employees in the fabrication shop were utilizing a Roller Pipe Cutter was not  guarded in a way to protect from point of operation and rotating part hazards.

29 CFR  1910.253(b)(4)(iii):Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet.  On or about July 17, 2025, employees in the fabrication shop were storing used oxygen and acetylene tanks together (less than 20 feet apart).

29 CFR  1910.305(g)(1)(iv):Flexible cords and/ or cables were used for purposes prohibited by paragraphs (A) through (F) of 29 CFR 1910.305(g)(1)(iv)(a).   On or about July 17, 2025, employees in the fabrication shop were utilizing a permanently fixed Roller Pipe Cutter that was powered by a flexible (extension) cord as a substitute for fixed wiring.
