29 CFR 1910.212(a)(3)(ii):The point of operation of machines whose operation exposes an employee to injury, were not guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. a) On or about June 27th, 2025 the employer Custom Hoists Inc. located at 771 CR 30A Hayesville, Ohio 44805 did not ensure the point of operation of the Cell 22 press was adequately guarded. There were no guarding methods at the rear end of the press, allowing employees to the access point of operation during a cycle and exposing them to crushing hazards.

Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. a) On or about June 27th, 2025 the employer Custom Hoists Inc. located at 771 CR 30A Hayesville, Ohio 44805 failed to contact the Toledo Area OSHA Office within 24-hours after an employee received injuries resulting in an amputation from a workplace injury involving Cell 22.
