29 CFR 1910.333(a): Safety-related work practices were not employed to prevent electric shock or other injuries resulting from either direct or indirect electrical contacts, when work was performed near or on equipment or circuits which were or could be energized. Location: Job site; On or about 07/17/2025, an employee was permitted to perform work inside a manhole (electrical vault) on 13.8 kV lines without verifying that all circuits or sources of voltage had been de-energized. The employee was exposed to the risk of electric shock and arc flash while working on or near equipment that was or could have been energized.

29 CFR 1910.335(a)(1)(i): Employees working in areas where there were potential electrical hazards were not provided with electrical protective equipment that was appropriate for the specific parts of the body that needed to be protected and for the work being performed. Location: Job site; On or about 07/17/2025, an employee was permitted to work on 13.8 kV electrical lines without wearing electrical protective equipment appropriate for the hands and arms. During the installation of a new cable into an existing energized line, an arc flash occurred, resulting in second-degree burns to the employee's right hand.
