Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  On or about July 17, 2025, at 200 S Main St. Mayville, WI 53050, employees engaged in roofing activities at heights greater than 6ft on a commercial building were not protected from a fall hazard. Employees replaced shingles without the use of fall protection and were exposed to bodily injury from an impact to a hard surface below.
