29 CFR  1926.416(a)(1): The employer permitted employees to work in close proximity to energized, unguarded, and ungrounded electric power circuits that the employee could contact in the course of their work: (a)(LOCATION)(IDENTIFY SPECIFIC EQUIPMENT AND/OR CONDITIONS)(DESCRIBE HAZARDS WHERE NECESSARY)  a) On or about July 14, 2025, at a worksite located at 2619 Wisconsin Ave NW, Washington, DC; parking deck, The employer did not properly de-energize the switchgear while employees were running cable, which led to an electrical arc. As a result, one employee sustained electrical burns and also experienced smoke inhalation.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.  This documentation may include but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR  1926.416(a)(3):The employer began work without determining if employees could contact any part of an energized circuit and did not post warning signs and advise employees of the location of the circuit, hazards, and protective measures they need to take.  (a)(LOCATION)(IDENTIFY SPECIFIC EQUIPMENT AND/OR CONDITIONS)(DESCRIBE HAZARDS WHERE NECESSARY)  a) On or about July 14, 2025, at a worksite located at 1776 K Street NW, Washington, DC, the employer failed to develop comprehensive safety procedures and did not conduct thorough training for employees on the hazards associated with working withing an energized switchgear high-voltage equipment (480V).  They  also failed to provide proper personal protective equipment (PPE), including insulated gloves, voltage-rated tools, and arc-rated clothing. As a result of these oversights, a serious incident occurred in which an employee suffered electrical burns from an electrical flash.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.
