29 CFR  1926.451(g)(1)(vii):Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  Location- 242 Main St, Millis, MA  On July 17, 2025, an employee was exposed to fall hazards while using a telehandler work platform as an elevated work surface approximately 20 feet above ground level without properly utilizing fall protection

29 CFR  1926.502(d)(15):Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person.  Location- 242 Main St Millis, MA  On July 17, 2025 an employee was exposed to falls when his vertical lifeline was not secured properly. The lifeline was wrapped around a truss and was not attached with a peak anchor, strap anchor or bolt-on anchor

29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee who might be exposed to fall hazards:  Location: 242 Main St, Millis, MA  On July 17, 2025, workers were exposed to fall hazards of up to 25 feet to the ground while conducting framing activities where fall protection was not properly used. Fall protection training was not provided to recognize and minimize fall hazards.
