(a) Kitchen: On July 17, 2025, and times prior to, employees were exposed to electrical shock and fire hazards while working near the Hatco Glo-Ray commercial food warmer (220 volts), GR.H60TB200, Model: GRH-60, S/N: 6016602518 in that the receptacle was not secured to the outlet box and was being pulled loose by the weight of the power cord.  (b) On July 17, 2025, and times prior to, the latch to the "Outside" electrical service panel door was broken.  (c) On July 17, 2025, and times prior to, the latch to the "B" electrical service panel door was broken and the door was held shut with wire.
