29 CFR 1910.178(n)(14): Traveling fork trucks did not avoid running over loose objects on the roadway surface. a) On or about July 17, 2025, and at time prior thereto, at the jobsite located at 3700 SW 47th Ave., Davie, FL 33314, a Toyota Forklift Model 8FD60U SN 11168 was driven over loose objects including but not limited to, shock absorber and vehicular equipment parts resulting in employee exposure to struck-by and crushed-by hazards.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least: a) On or about July 17, 2025, and at time prior thereto, at the jobsite located at 3700 SW 47th Ave., Davie, FL 33314, employees used compressed oxygen and fuel gas cylinders. The employer did not ensure that the Oxygen compressed gas cylinders were separated when not in use. This condition exposed employees to fire, burns and struck-by hazards. The SA Recycling LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (name previously cited standard), which was contained in OSHA inspection number 1671625, citation number 1, item number1, and was affirmed as a final order on December 15, 2023, with respect to a workplace located at 6955 FM 511 Cameron, TX 78526.

29 CFR 1910.101(a): The employer did not determine that compressed gas cylinders under his control were in a safe condition to the extent that can be determined by visual or other inspections conducted as prescribed in the Hazardous Materials Regulations of the Department of Transportation (49 CFR parts 171-179 and 14 CFR part 103). Where those regulations are not applicable, visual and other inspections shall be conducted in accordance with Compressed Gas Association Pamphlets C-6-1968 and C-8-1962, which is incorporated by reference as specified in Sec. 1910.6. a) On or about July 17, 2025, at the jobsite located at 3700 SW 47th Ave., Davie, FL 33314, the employer did not determine the regulator gauge was in good condition, while an employee was utilizing a regulator for acetylene; its glass cover was missing for one of the gauges.
