29 CFR 1926.605(d)(2):The employer shall ensure that there is in the vicinity of each barge in use at least one U.S. Coast Guard-approved 30-inch life ring with not less than 90 feet of line attached, and at least one portable or permanent ladder which will reach the top of the apron to the surface of the water. If the above equipment is not available at the pier, the employer shall furnish it during the time that he is working the barge. On or about July 18, 2025 in Laconia , NH Barges were not equip with a U.S. Coast Guard-approved 30 inch life ring with not less than 90 feet of attached line or a ladder to reach the top of the apron to the water surface.

The employer did not have adequate provisions for rendering first aid and medical assistance in that: 1926(50)(c) first aid training was not provided to employees, 1926(50)(d)(2) first aid kits were not inspected at least weekly.
