29 CFR 1910.147(c)(4)(i): The employer did not develop, document, and utilize procedures for the control of potentially hazardous energy when employees are engaged in activities covered by this section: (a) On July 16, 2025, at Moody 159-94-15A-22-1H Wellsite near Battleview, North Dakota, employees were exposed to struck-by and caught-in-between hazards that could result in serious injuries from the release of hazardous stored energy such as, but not limited to hydraulic and mechanical energy. Employees were required to perform maintenance activities such as, but not limited to, replacing a belt on an electric motor on the pumping unit without having or utilizing procedures to control all hazardous energy sources.

29 CFR 1910.147(d): The application of energy control (lockout or tagout) actions were not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): (a) On July 16, 2025, at Moody 159-94-15A-22-1H Wellsite near Battleview, North Dakota, employees are exposed to struck-by and caught-in-between hazards that can result in serious injuries from the release of hazardous stored energy such as, but not limited to hydraulic and mechanical energy. The employer did not follow methods of control for [1910.147(d)(1)]; orderly shutdown [1910.147(d)(2)]; locating/operating energy isolating devices [1910.147(d)(3)]; lock-out/tag-out device application [1910.147(d)(4)]; dissipation of residual energy [1910.147(d)(5)]; and verification of isolation [1910.147(d)(6)] were not implemented prior to maintenance and service activities.
