29 CFR  1926.300(b)(3): One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  5528 & 5578 Tamarack Ave., Firestone CO: On or about July 17, 2025, the company's 4.5-inch, DeWalt right angle hand grinder (Model #: DWE402) was used without an affixed guard. Employees using the unguarded hand grinder and others in the immediate area were not protected from hazards created by flying chips and sparks and/or struck-by hazards from the abrasive wheel if it were to break.

29 CFR  1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:  5528 & 5578 Tamarack Ave., Firestone CO: On or about July 17, 2025, the employer failed to ensure that the company's 12-gauge (flexible) extension cord use to power handheld tools such as but not limited to, a Dewalt right hand angle grinder was free from damage. The outer sheath was cut exposing internal wiring and attempts at repair, were done using electrical tape. Employees using the extension cord were not protected from hazards created by inadvertent/accidental contact with live electrical parts.

29 CFR  1926.451(c)(2)(i): Footings were not level, sound, rigid, and capable of supporting the loaded scaffold without settling or displacement:  5528 & 5578 Tamarack Ave., Firestone CO: On or about July 17, 2025, tubular steel scaffolding used by employees engaged in masonry work was not placed on solid and stable ground. The employer did not ensure the surface ground area was properly graded causing the scaffolding to lean approximately 2 degrees. The unleveled scaffold was not properly supported by a base plate.

29 CFR  1926.1153(c)(1): For each employee engaged in a task identified on Table 1, the employer did not fully and properly implement the engineering controls, work practices, and respiratory protection specified for the task on Table 1, unless the employer assesses and limits the exposure of the employee to respirable crystalline silica in accordance with paragraph (d) of this section:  5528 & 5578 Tamarack Ave., Firestone CO: On or about July 17, 2025, the employer did not ensure that employees were protected from respirable crystalline silica hazards while cutting concrete. Respirators and/or water to dissipate plumes of respirable crystalline silica dust were not introduced into the workplace
