Employees were exposed to impalement hazards while replacing a regulator and valves near unguarded reinforcing steel.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: Location: Underhill Rd & Clayton Rd, Scarsdale, NY 10583 a) On or about July 21, 2025, employees used a metal extension ladder for trench access and egress and the side rails of the ladder did not extend at least 3 feet above the trench. Employees were exposed to fall hazards of approximately 6'4".
