Worksite: On or about July 21, 2025, the employer did not ensure a personal fall arrest system anchor was capable of supporting a load of 5,000 pounds and properly installed per manufacturer's instructions resulting in two employees suffering injuries. The employees were working on a roof at a height of approximately 20 feet, 9 inches from ground level.

Worksite: On or about July 21, 2025, the employer did not ensure anchorages used for attachment of personal fall arrest equipment were designed, installed, and used under the supervision of a qualified person. A supervisor and an employee were tied in to the same 5000 lb. anchor which failed causing both employees to fall to the ground.

Worksite: On or about July 21, 2025, the employer did not inspect personal fall arrest systems prior to use for wear, damage, and other deterioration, removing all components from service prior to employee use. One of the lifelines in use was missing a grommet that protects the rope from abrasions from the metal snap hook.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards. Worksite: On or about July 21, 2025, the employer did not ensure that employees who were engaged in residential roofing at heights above six feet were trained in fall hazards and the procedures to be followed to minimize such hazards. The two untrained employees fell from a residential roof when an anchor failed that they were both tied into.
