29 CFR 1910.333(c)(3)(iii)(A): Vehicles or mechanical equipment capable of having parts of their structures elevated near energized overhead lines were not operated so that a clearance of 10 feet (305 cm) was maintained. On or about July 11, 2025, and at times prior to, an employee was operating the boom of a vacuum truck, to clean a storm drain, less than 10 feet from an overhead powerline.

29 CFR 1910.332(b)(2): Employees, who were not qualified persons, were not trained in and familiar with any electrically related safety practices not specifically addressed by 29 CFR 1910.331 through 29 CFR 1910.335, but which were necessary for their safety. On or about July 11, 2025, and at times prior to, an employee was operating a Vactor vacuum truck under high voltage powerlines while cleaning out storm drains without being trained in electrical safe work practices.
