(a) 1011 Morris Ave, Suite B3-C, Union, NJ 07083  On July 17, 2025, an employee incurred burns to their left hand while cleaning filters in the fog tank, which is filled with water at temperatures between 170-180 degrees Fahrenheit. The employee was wearing cloth oven gloves while completing this task.  Condition(s) occurred on July 17, 2025 and thereafter.

29 CFR  1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  (a) 1011 Morris Ave, Suite B3-C, Union, NJ 07083  On or about July 21, 2025, suitable facilities were not provided within the work area for the immediate emergency flushing of both eyes of employees who use chemical cleaners such as Kay QSR Restroom Cleaner, which the Safety Data Sheet (SDS) and container label indicate as corrosive and recommend rinsing with plenty of water in case of eye contact.  NOTE:  American National Standards Institute (ANSI) Standard Z358.1 criteria for emergency eyewash is one capable of delivering not less than 0.4 gallons (1.5L) of clean water per minute continuously for at least 15 minutes
