29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:    At a worksite located at 6110 Rockhill Road, Kansas City, MO 64110:  Employees were exposed to fall hazards. This was last demonstrated on or about July 11, 2025, when workers were working at height over 6 feet near unprotected sides and edges of the roof. The employer failed to protect employees from falling by not requiring employees to use either guardrail systems, safety net systems, or personal fall arrest systems.

29 CFR  1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:  At a worksite located at 6110 Rockhill Road, Kansas City, MO 64110:  Employees were exposed to fall hazards. This was last demonstrated on or about July 11, 2025, when employees were using extension ladder to gain access to the roof of a residential building. The employer failed to protect employees from falling by not ensuring that employees using the ladder placed the ladder so that it extended at least 3 feet above the landing being accesses and further the ladder was not secured at its top to a rigid support that will not deflect. The ladder only extended 1.5 to 2 feet above the landing and was not secured.

29 CFR  1926.1053(b)(5)(i): Non-self-supporting ladders were not used at an angle such than the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support):  At a worksite located at 6110 Rockhill Road, Kansas City, MO 64110:  Employees were exposed to fall hazards. This was last demonstrated on or about July 11, 2025, when employees were using extension ladder(s) to gain access to the roof of a residential building. The employer failed to protect employees from falling by not ensuring that employees using the ladder placed the ladder so that the angle of the ladder was approximately one-quarter of the working length of the ladder. The ladder angle was too shallow.
