Location: 201 Lyons Avenue, Newark , 07112 a) The employer failed to report within 24 hours an inpatient hospitalization when an employee was admitted to the hospital on 07/02/2025 after an employee sustained head injury from an aggressive patient. The incident was reported to OSHA on July 14, 2025. Violation occurred on or about July 3, 2025
