This failure exposed employees to amputation hazards. On or about 07/16/25.

Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. A) 20 Jernee Mill Road, Sayreville, NJ 08872 The employer failed to notify the Occupational Safety and Health Administration (OSHA) of an amputation within the required 24-hour timeframe. An employee suffered a finger amputation on 07/16/2025, but the employer did not report the incident to OSHA until 07/20/2025. On or about July 16, 2025.
