29 CFR 1910.269(l)(4)(i): The employer did not ensure that when an employee uses rubber insulating gloves as insulation from energized parts (under paragraph (l)(3)(iii)(A) of this section), the employee shall also use rubber insulating sleeves: On or about July 17, 2025, an employee's arm made contact with a 120 volt secondary circuit on a pad transformer that was not deenergized and the employee was not using rubber insulating sleeves.
