29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system: On or about July 22, 2025, Employees were exposed to fall hazards in approximately 14 feet height while performing residential construction activities from a ladder jack. No fall protection was being utilized while employees were standing on the ladder jack in the process of installing windows . No abatement certification or documentation is required for this item.

29 CFR 1926.454(a)(1): The employer failed to have each employee who performs work while on a scaffold trained regarding the nature of any electrical hazards, fall hazards and falling object hazards in the work area. On or about July 22, 2025, the employer did not ensure that employees had been trained to recognize and minimize fall hazards. Employees were exposed to fall hazards in approximately 14 feet while performing window installation activities from a ladder jack scaffold on a residential structure without using fall protection. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
