On or about July 23, 2025, workers were permitted to operate an unguarded WDM Metal Roller (Model B-3.5-48, SN 61482-17). The employer did not ensure point of operation hazards were guarded. b) Fulton Thermal Corporation, 972 Centerville Road, Pulaski, NY - Thermal Area. On or about July 23, 2025, workers were permitted to operate Marvel Mark II Vertical Bandsaw #3, while exposed to the unguarded portion of the saw. c) Fulton Thermal Corporation, 972 Centerville Road, Pulaski, NY - Thermal Area.
