On or about July 23, 2025, at the above addressed jobsite, an employee was exposed to fall hazards while performing painting activities from a platform plank supported by two tubular welded frame scaffolds that were not inspected by a competent person.

On or about July 23, 2025, at the above addressed jobsite, an employee was exposed to fall hazards while performing painting operations from a platform plank supported by two tubular welded frame scaffolds that were not altered by a trained and experienced employee under the supervision and direction of a competent person qualified in scaffold alteration.

On or about July 23, 2025, at the above addressed jobsite, an employee performing painting activities from a platform plank supported by two tubular welded frame scaffolds, twenty (20) feet nine (9) inches above ground, was not protected from falls, thereby exposing the employee to fall hazards.
