29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  26 Glen Street, Westborough, MA: On or about July 24, 2025, employees were observed working on a residential structure engaged in roofing activities with unprotected sides and edges greater than 6 feet above the next lower level and were not protected by either guard rails, safety net systems or personal fall arrest systems exposing them to fall from height hazards.  LESLEY CONSTRUCTION INC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.503(b)(13) , which was contained in OSHA inspection number 1816779, citation number 1, item number 2, group letter a and was affirmed as a final order on 04/25/2025, with respect to a workplace located at 22 Franklin St, Braintree, MA.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards:   26 Glen Street, Westborough, MA: On or about July 24, 2025 and at times prior employee(s) were exposed to fall hazards as part of their assigned duties and the employer had not trained them prior to exposure to these hazards in topics such as but not limited to recognition of fall hazards, dangers of fall hazards and procedures each employee must take to prevent or minimize risk of such exposures.  LESLEY CONSTRUCTION INC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.503(a)(1) , which was contained in OSHA inspection number 1816779, citation number 1, item number 2, group letter b and was affirmed as a final order on 04/25/2025, with respect to a workplace located at 22 Franklin St, Braintree, MA.  Luis Pinal dba Lesley Roofing Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.503(a)(1) , which was contained in OSHA inspection number 1632190, citation number 1, item number 4, group letter a and was affirmed as a final order on 02/08/2023, with respect to a workplace located at 43 N. Quisigmond Ave., Shrewsbury, MA.
