29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a) Roof - On or about July 23, 2025 and at times prior thereto, an employee working on the gutters was exposed to falls while installing gutters from a 17 foot 6 inch high roof without fall protection.  No fall protection means was used by employee.

29 CFR  1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a) Roof - On or about July 23, 2025 and at times prior thereto, an employee working on the gutters was accessing the roof of the building with an extension ladder that extended approximately 1 foot above the landing surface.

29 CFR  1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer:  a)  Roof - On or about July 23, 2025 and at times prior thereto, an employee was exposed to falls while installing gutters from a 17 foot 6 inch high roof without fall protection.  The employee had received training but the employer could not verify training with any certification.
