This condition exposed employees to struck-by hazards associated with improper forklift operations. (b)Callies Homes, Inc, at 1011 Putter Circle, Dell Rapids, SD 57022: On or about and at times prior to July 21, 2025, the employer did not ensure that all PIT operators were competent to operate the Ingersoll-Rand Telehandler (Middle), Model VR-1056C, safely by a successful completion of a training program. This condition exposed employees to struck-by hazards associated with improper forklift operations. 29 CFR 1926.451(c)(2)(iv): The employer did not ensure front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use: (a)Callies Homes, Inc, at 1011 Putter Circle, Dell Rapids, SD 57022: On or about and at times prior to July 21, 2025, the employer did not ensure that the custom work platform used in conjunction with the all terrain Gradall G6-42A telehandler, serial number 0190363, was designed, constructed, or used in accordance with the requirements of the rough terrain forklift manufacturer while conducting siding operations on a residential home. The platform was not specifically approved by the forklift manufacturer nor did it meet ASME/ITSDF B56.6-2011 requirements. The custom made platform exceeded criteria for the overall length and did not have a data plate identifying maximum working load including personnel and equipment. This condition exposed employees to fall hazards and crushing hazards related to the platform separating from the mast or tipping. Abatement Note: Among other methods, one feasible and acceptable abatement method to correct this hazard is to ensure that all platforms used for elevating personnel with a rough terrain forklift truck comply with the design requirements specified in ASME/ITSDF B56.6-2021, American Society of Mechanical Engineers/Industrial Truck Standards Development Foundation, "Safety Standards for Rough Terrain Forklift Trucks", Section 2, paragraph 5.17 Elevating Personnel, as well as Section 3, paragraph 8.24, Personnel Work Platforms for Elevating Personnel. The ANSI/ITSDF B56.6-2021 standard can be obtained by contacting the American National Standards Institute, 1899 L Street, Northwest, 11th Floor, Washington, DC 20036.

This condition exposed employees to fall hazards and crushing hazards associated with the platform separating from the mast of the forklift. Employees were exposed to fall hazards from telehandler platforms, standing on platforms not secured to other elevated platforms, ladder hazards, telehandler platforms not secured to the mast, as a result of conducting residential siding activities. This condition exposed employees to a variety of fall hazards identified on site.
