29 CFR  1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered:  (a)Green Light Remodeling & Construction, LLC, at 133rd Street and 395th Avenue, Bath, SD 57427: On or about and at times prior to July 22, 2025, the employer did not ensure the JLG Model: G6-42A forklift was marked for use with the approximately 15 foot long homemade boom lift attachment.  This condition exposed employees to struck-by hazards.  (b)Green Light Remodeling & Construction, LLC, at 133rd Street and 395th Avenue, Bath, SD 57427: On or about and at times prior to July 22, 2025, the employer did not ensure the JLG Model: G6-42A forklift was marked for use with the approximately 30 foot long homemade splitter attachment.  This condition exposed employees to struck-by hazards.

29 CFR  1910.178(l)(1)(i): The employer did not ensure that each Powered Industrial Truck (PIT) operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):  (a)Green Light Remodeling & Construction, LLC, at 133rd Street and 395th Avenue, Bath, SD 57427: On or about and at times prior to July 22, 2025, the employer did not ensure that all PIT operators were competent to operate the JLG All-Terrain Forklift, Model: G6-42A, safely by a successful completion of a training program.  This condition exposed employees to struck-by hazards associated with improper forklift operations.  (b)Green Light Remodeling & Construction, LLC, at 133rd Street and 395th Avenue, Bath, SD 57427: On or about and at times prior to July 22, 2025, the employer did not ensure that all PIT operators were competent to operate the Bobcat, Model: T770, safely by a successful completion of a training program.  This condition exposed employees to struck-by hazards associated with improper forklift operations.

29 CFR  1926.451(g)(1):The employer did not ensure each employee on a scaffold more than 10 feet above a lower level was not protected from falling to that lower level:  (a)Green Light Remodeling & Construction, LLC, at 133rd Street and 395th Avenue, Bath, SD 57427: On or about and at times prior to July 22, 2025, the employer did not ensure that employees were protected from falls while allowed to stand from the guard rail protective system on the Geni Scissor Lift, Model: GS-3268RT while conducting truss setting operations on a commercial pole barn.  This condition exposed employees to fall hazards greater than 10 feet.  (b)Green Light Remodeling & Construction, LLC, at 133rd Street and 395th Avenue, Bath, SD 57427: On or about and at times prior to July 22, 2025, the employer did not ensure that employees were protected from falls while allowed to stand from the guard rail protective system on the Skyjack Scissor lift, Model: SJ6826, while conducting truss setting operations on a commercial pole barn.  This condition exposed employees to fall hazards greater than 10 feet.

This condition exposed employees to a fall hazard of 10 feet.  (b)Green Light Remodeling & Construction, LLC, at 133rd Street and 395th Avenue, Bath, SD 57427: On or about and at times prior to July 22, 2025, the employer did not ensure that employees were trained on the use of mobile scaffolding systems prior to working from and operating the Skyjack scissor lift Model number:  SJ6826 on a commercial pole barn site.
