On a driveway, at a temporary worksite located at 77 Tordera Lane, Saint Augustine, FL: On or about July 24, 2025, the employer exposed employees to respiratory hazards, in that employees were required to wear a NIOSH approved, 3M brand, half facepiece respirator while cutting pavers containing silica dust and mixing Portland cement and the employer did not establish and implement a written respiratory protection program that addressed the worksite-specific procedures to cutting operations.
